Health Assessments in North Lakes
A health assessment is a structured, unhurried check of your health with your GP and our practice nurses — your history, examinations and any investigations you need, followed by a clear plan for staying well. Medicare funds health assessments for specific groups of patients, and Best Life Medical Centre offers them to all eligible patients.
Who is eligible for a Medicare health assessment?
- People aged 45–49 who are at risk of developing a chronic disease (once only)
- People aged 75 and over (every year)
- People aged 40–49 at high risk of type 2 diabetes (every three years)
- Permanent residents of residential aged care facilities (every year)
- People with an intellectual disability (every year)
- Refugees and humanitarian entrants (once only)
- Former serving members of the Australian Defence Force (once only)

What happens at a health assessment
Our practice nurse usually begins by collecting your history and routine measurements, and your GP then completes the assessment with you — examining anything that needs attention, arranging investigations if required, and putting together a written plan with recommendations for your health. It is a longer appointment than a standard consultation, so please book it as a health assessment.
If the assessment picks up a condition that needs ongoing care, your doctor may recommend a chronic condition management plan. Registering with MyMedicare and choosing us as your regular practice also helps us provide better-connected care.
