Chronic condition management plans
If you have a chronic condition that has been present, or is likely to be present, for six months or more, your doctor may recommend a GP Chronic Condition Management Plan — the Medicare arrangement that replaced GP Management Plans and Team Care Arrangements from 1 July 2025. A plan sets out your health goals, the care you need and who provides it, and can give you Medicare-supported access to certain allied health services such as podiatry, physiotherapy and dietetics. Your doctor will review the plan with you regularly to keep it up to date.
